About me

Lily Uzzaman is a primary care nurse practitioner with interests in preventive health care, well-child checkups, contraception, gender-affirming care, acute and chronic medical conditions, and common mental health conditions like anxiety, depression and ADHD. She practices trauma-informed care and believes in shared decision-making and in honoring the stories and autonomy of her patients in order to help them navigate the health care system to achieve their best health.

Lily comes from a family of foodies, so in her free time she loves to cook, drink coffee and find the best pastries in the city. She attends many of her kids’ sports and school events, enjoys trail running and visits to the coast, and listens to as many podcasts as she can.

Insurance

Before scheduling your appointment, we recommend you contact your insurance company to verify that the OHSU Health location or provider you plan to visit is included in your network. Your insurance company will also be able to inform you of any copayments, co–insurance, or deductibles that will be your responsibility. If you proceed in scheduling an appointment and your health insurance benefits do not participate with OHSU providers, your out of pocket liability may be higher. Visit the billing and insurance section of our site for more information.
