Directed Sperm Donor
A directed donor is known by the recipient but is not her sexually intimate partner. Directed sperm donation falls under the FDA regulation of tissue products. To meet these regulations and our own internal policies and procedures, it is necessary to cryopreserve the semen and to test and screen the donor on the day of, or within 7 days before or after, the first donation. After screening and testing is completed (approximately one week), University Fertility Consultants (UFC) reviews testing and screening results and determines whether the donor is eligible or ineligible. Under some circumstances, samples from ineligible donors may be released for use after notifying the recipient of the ineligible status and obtaining the physician signature on an ineligibility notification form. Physicians at UFC are pleased to offer directed sperm donation as an option to their patients and will provide a written order to the OHSU Andrology Lab to begin the process.
Important Note: The UFC only uses directed donor samples obtained within the 7 day window of testing and screening as permitted by the FDA. If donors wish to collect additional samples outside the 7 day window then they must be completely rescreened and retested again to initiate another one-week period of donations.
Relevant communicable disease testing and screening
Testing is to be performed within seven days before or after freezing the first semen sample but preferably on the same day.
Tests on blood

- Hepatitis B surface antigen
- Hepatitis B core antibody - total
- Hepatitis C virus antibody
- HTLV - I and II
- RPR
- Cytomegalovirus (CMV) total antibody (see physician for interpretation of a positive result.)
- Cytomegalovirus (CMV) antibody - IgM (only required if CMV total antibody is positive.)
- West Nile virus
- Zika virus
Tests on urine
- Chlamydia trachomatis
- N. gonorrhea
Physical examination
Donors must obtain an OHSU Andrology Lab Donor Physical Examination form to give to the physician for him/her to complete. Any physician may conduct the exam. Two providers at OHSU Urology, Jason Hedges, M.D., and Matthew Keeslar, P.A.-C., are specialists in male fertility and advisors to the Andrology Lab donor program. They are available to perform FDA-required physical examinations.
Medical history interview
Prior to the physical exam, the OHSU Andrology Lab will conduct a 10-15 minute risk factor interview.
Psychological evaluation
Prior to testing, screening, and sample collection, every directed sperm donor and his recipient if under the care of a UFC physician must have an appointment with a qualified psychosocial counselor with experience in fertility. They can meet together with the counselor or individually.
Cryopreservation of semen
Samples may be frozen at the OHSU Andrology Lab. If the donor desires to use a different FDA compliant laboratory for cryopreservation, then that laboratory must agree to conduct all screening, testing, and FDA donor eligibility determination.
