Ordering Transcripts and Other Documents

Transcript and document fees

Transcript fee: $20
MSPE (Dean's letter) fee: $10
Certified copy of diploma fee: $10

Ordering documents

To order an official transcript:

  1. Log in to Parchment Transcript Services website.
  2. Enter your email address and press 'Continue.'
  3. Follow the instructions to place an order. You will update your contact information, select a transcript recipient, and enter payment information.

**You will be given the option to select when you want your order sent:

  • Send Now - your order will be released immediately
  • Hold for Degree - your order will be released once your degree is conferred 
  • Hold For Grade - your order will be released once your current term grade(s) are posted

Residency Application Transcript Requests:

For your first Residency Application Service transcript  (ERAS, Military, CAS, Ophthalmogogy, Residency CAS), complete the  School of Medicine Residency Transcript Request Form below and send it to Kaitlin Seymore.

For subsequent Residency Application requests, order your transcript through Parchment as described above, select OHSU and Kaitlin Seymore  as the recipient. Kaitlin will upload your transcript and MSPE (Dean’s letter) to the Residency Application portal. You do NOT need to order your MSPE through Parchment. 

To order a Certified Copy of Diploma:

  1. Log in to Parchment Transcript Services website.
  2. Enter your email address and press 'Continue.'
  3. Follow the instructions to place an order. You will update your contact information, select a recipient, and enter payment information.

When ordering a Certified Copy of Diploma, you will need to provide a copy for certification UNLESS you are a:

  • Graduate from Spring 2023 and forward
  • DMD graduate since 2011
  • MD graduate since 1996

Copies or scans of diplomas can be emailed or faxed to the Registrar's Office at regohsu@ohsu.edu or 503-494-4629.

To order a Replacement Diploma:

  1. Log in to Parchment Transcript Services website.
  2. Enter your email address and press 'Continue.'
  3. Follow the instructions to place an order. You will update your contact information, select a recipient, and enter payment information.

For records related to your residency or fellowship, contact the Graduate Medical Education (GME) office at 503-494-8513 or at gme@ohsu.edu.

Unofficial Transcripts

Students with an electronic record can view their unofficial transcripts online in Student Self-Service. Please note: all records are electronically available. Some students may have to request an official transcript, using Parchment, depending on when they attended (MD graduates before 1996, DMD graduates before 2011, and Oregon Graduate Institute and School of Science & Engineering before 2003).
