Pre-hire requirements: Occupational Health onboarding
The Occupational Health team at OHSU is committed to the health, safety and well-being of OHSU employees and volunteers.
As you prepare to join OHSU, please review the information on this page and complete the required steps prior to your first day of work.
All new employees need to complete items 1 and 2, regardless of position:
Within 1-2 days after accepting your written offer letter, you will receive an email with instructions and an activation code for setting up an account in Enterprise Health, OHSU’s Occupational Health system. Please watch for an email from <DoNotReply@ohsu.edu> with the subject line “Portal activation instructions” (and please be sure to check your junk mail folder).
Follow the instructions in the email for “New user registration” to activate your Enterprise Health account. If you need assistance, email occhealth@ohsu.edu.
OHSU strongly recommends that all OHSU employees, regardless of your work location or job function, be up to date with COVID-19 vaccination.
Once you have activated your Enterprise Health account (see 1, above), follow these instructions to upload proof of your full vaccination record. It is important that you complete this upload prior to your first day of work.
If you are not up to date with COVID-19 vaccination and choose to decline vaccination, you must complete and submit a declination form. Follow these instructions to submit your declination form through your Enterprise Health account.
Complete items 3, 4 and 5 only if the position you are being hired into requires any of the following:
- You will have direct patient contact.
- You will work in a building where patients are seen.
- You may have contact with blood and potential exposure to blood-borne pathogens.
- You will work at the West Campus Primate Center, Vaccine and Gene Therapy Institute (VGTI), or your work will involve access to animals.

Responses to these surveys help us identify potential risks and the appropriate tests, immunizations and other procedures to keep you healthy and safe while at work.

Once you have activated your Enterprise Health account (see 1, above), follow these instructions to self-schedule your appointment. You can schedule an appointment to take place before your first day or after. If there are only dates available after you start, we ask that you select the earliest date available.

- Safety Compliance Services that are performed outside of Occupational Health and required for your role are eligible for reimbursement.
During your onboarding appointment, Occupational Health staff will do their best to address any safety compliance requirements needed for your position. This is to ensure your overall health and safety in your workplace environment. These services may include: historical record review, TB screening, respirator fit testing, immunizations, titers (blood draws), and/or clearances.
