OHSU Hospital and Clinics laboratories are accredited by the College of American Pathologists (CAP), DNV-GL, the Oregon Health Authority, or other approved agencies.  The laboratories also maintain current CLIA certification by Centers for Medicare and Medicaid Services (CMS). All testing activities for OHSU Laboratory Services and OHSU Laboratory Services at CHH will be merged into one CLIA and one CAP. 
The CHH CLIA/CAP certificates will be discontinued, and all testing activity will be combined into the following CLIA and CAP numbers.
