Get Connected to Preventive Care That Works For You

Eligibility for Linkage Project candidates: 

  • have I/DD

  • 18 years or older 

  • live in Oregon 

  • able to connect with Linkage Project Team on Zoom or phone in English or Spanish

  • have a specific unmet preventive health need (PDF linked here for examples)

  • want to increase their health knowledge and get connected to resources  

The Linkage Project is for adults with intellectual and developmental disabilities (I/DD) who are not getting their preventive health needs met.

The Linkage Project Team from the Oregon Office on Disability and Health helps in three ways:   

  1. They ask people in the project questions about their preventive health needs.  
  2. They help people learn about healthy behaviors, going to the doctor, going to the dentist, getting screening tests, and getting mental health counseling
  3. They help people find preventive health care and other health supports in their community.
