Get Connected to Preventive Care That Works For You
Eligibility for Linkage Project candidates:
-
have I/DD
-
18 years or older
-
live in Oregon
-
able to connect with Linkage Project Team on Zoom or phone in English or Spanish
-
have a specific unmet preventive health need (PDF linked here for examples)
-
want to increase their health knowledge and get connected to resources
The Linkage Project is for adults with intellectual and developmental disabilities (I/DD) who are not getting their preventive health needs met.
The Linkage Project Team from the Oregon Office on Disability and Health helps in three ways:
- They ask people in the project questions about their preventive health needs.
- They help people learn about healthy behaviors, going to the doctor, going to the dentist, getting screening tests, and getting mental health counseling
- They help people find preventive health care and other health supports in their community.
