How to become a Partnership Project client
Eligibility
Anyone living with HIV in these counties is eligible to become a Partnership Project client:
- Multnomah County
- Clackamas County
- Washington County
- Yamhill County
- Columbia County

Case management
Case management
Case management service is designed to meet the needs of people living with HIV and their families who need intensive, ongoing and regular contact to coordinate needed services. Clients receive long-term care coordination in relation to a variety of jointly identified issues such as housing, insurance, mental health, substance abuse, and non-HIV related health problems. Clients assessed to need Nurse Case Management are provided assistance monitoring their medication, health and coordination of medical care.
Clients can leave services when their goals have been accomplished and can also return to services if needed.
Services are provided in a culturally and linguistically appropriate manner. We have two staff members who are fluent in Spanish. We also contract with the Immigrant and Refugee Community Organization (IRCO) to provide interpreters for clients fluent in languages other than Spanish.

Nurse case management
Nurse case management
Nurse case management is designed to meet the needs of clients who are experiencing the following: recent or current hospitalization; medical adherence issues; significant changes in health status or ability to manage activities of daily living; pediatric clients; multiple health and mental health diagnosis and end-of-life needs.
Nurse Case Managers are assigned to a health system or group of health systems to facilitate the best coordination with case managers, clinic nurses and medical providers.
All clients who receive an intake are assessed for their need for Nursing Case Management. Clients can also be referred by clinic nurses, case management staff, and medical providers.
