About Care Management Plus
Founded in 2005 by Dr. David Dorr in conjunction with Intermountain Healthcare, the objective of Care Management Plus (CMP) began as a technology-intensive, innovative primary care model that reduces hospitalizations, lowers costs, and improves health outcomes for older adults with chronic illnesses. The team continues to focus on interdisciplinary efforts to improve the quality of care for patients with chronic illnesses using care managers and information systems.
The evolving mission of the team is to better understand how data, information, and knowledge can assist in transforming health for our most vulnerable populations.
The CMP team of physicians, researchers, data analysts, and developers are currently working on eight collaborative projects. David Dorr, MD, MS serves as OHSU's Chief Research Information Officer (CRIO) and CMP team director. MJ Dunne, MA serves as the CMP team's program manager. The CMP team is based at the OHSU South Waterfront campus.
