If you forget your OHSU MyChart username or password, select "Forgot username?" or "Forgot password?" below the "Sign in" button on a laptop. On the mobile app, you can click "Need help?"

Steps to recover username

If you forget your username, you will be asked to verify your first name, last name, date of birth and ZIP code. Once you click the submit button, an email from mychart@ohsu.edu  will be sent to your email containing your MyChart username. The email subject will say “MyChart Verification code."

Steps to recover password

If you forget your password, you will need to verify your username, date of birth and ZIP code.

You will then be asked to enter a one-time security code that is 6 characters long. The code can be sent as either a text message or email. The code is only valid for 30 minutes.

Enter the code that was sent to you in the "enter code" box and select next.

The final step is to create and verify (retype) a new password. Once you push the "next" button, you will be taken to your homepage in your MyChart account.
