This session will provide a brief overview of the 4Ms framework of age-friendly care. The evidence-based 4Ms framework focuses on What Matters, Medication, Mentation, and Mobility. The 4Ms framework has shown positive outcomes throughout the participating health systems, with continued improvement expected. This session will focus on community health workers (CHWs) and how CHWs can incorporate the framework into practice.
