How to order and schedule imaging

You can use the order forms below to request imaging for your patient.

If your patient needs:

• Oral anxiolytics: Please order them from an OHSU pharmacy for the patient to pick up before their imaging appointment.
• IV anxiolytics or anesthesia: Please note on the order form.

Your patient can call to schedule an appointment at one of our Portland-area locations. In some cases, they may be able to schedule through MyChart, using the “Schedule an Appointment” tab.

If your patient is new to OHSU or their insurance has changed since their last visit to OHSU, they’ll need to call Patient Registration: 503-494-8505 or 888-222-6478.

Imaging appointments

We ask that your patient bring an adult to their imaging appointment if:

• They will have anxiolytics or anesthesia. The adult will need to drive or accompany them home.
• They are bringing a child or service animal. The adult will need to stay with the child or animal.
