Community Partners
Established in 2013 I-CAN works with local partner agencies in urban and rural communities across Oregon. Through home visits, nursing student teams work with clients over a period of time to identify goals and develop a care plan. Many plans are focused on navigating the health care system, increasing health awareness, and connecting clients with available resources within your community.
Typical I-CAN routine visits include up to two hours of confidential weekly meetings with the client’s assigned OHSU Nursing Student care team. Meetings may occur in the home, medical office, agency or public place of the client’s choice.
Students help with getting dental, mental and medical care for I-CAN clients. Along with help scheduling appointments and providing nutrition education. Students connect clients with community programs for food, housing and bill assistance. The I-CAN program supports clients by:
- Setting and achieving goals.
- Identifying concerns.
- Answering questions about health care visits.
- Helping to arrange medical transport and access to safety devices.
- If needed, interpreter services are easily available.
