OHSU’s Eugene Child Development and Rehabilitation Center evaluates and cares for children and young adults with developmental needs and chronic conditions.

Our clinics include:

• Child Development Clinic
• Feeding and Nutrition Clinic
• Neurodevelopmental Clinic
• Transition Clinic

Our team:

• Cares for patients up to age 21 (age 24 for Transition Clinic).
• Does outreach clinics 10 times a year in Medford and twice a year in Klamath Falls.
• Has longtime relationships with community providers.
• Works with partners to help children close to home:
  • Oregon Center for Children & Youth with Special Health Needs, which helps families get low-cost health care and services.
  • Care Coordination (CaCoon), which connects families with registered nurses for home visits.

When your child gets a referral to Eugene CDRC, you’ll need to fill out an intake packet:

• Intake Packet (below age 5)

• Intake Packet (age 5+)
  Spanish: Paquete de admisión para niños mayores

Once we get your intake packet, we’ll put your child on a waiting list for an appointment. We must have a complete intake packet to add a child to our waiting list.
