Immunization Requirements for Students

  • Tetanus series and booster
  • Measles (Rubeola)

*Begin your Hepatitis B immunization series as soon as possible. It can take up to 7 months to complete if you’re starting from scratch.

Submitting documentation

Before starting at OHSU, you must meet the Centers for Disease Control and Prevention (CDC) guidelines for health care workers.  

  • Check with your program for immunization deadlines. 
  • Fill out your Immune Status Form.  
  • Return the form and copies of documentation to the Student Health and Wellness Center by emailing shwcompliance@ohsu.edu.

What to expect if you don’t complete requirements by your program’s deadline

  • Complete your requirements within 14 days of starting your program to avoid a $95 fee. 
  • Complete your requirements within 30 days of starting your program to avoid a hold on your student account, which will suspend your student status and prevent you from registering for and attending classes. 

Immunization exemptions

If you have a medical condition and need an immunization exemption, complete a vaccine exemption form and submit it to the Student Health and Wellness Center for review. You must include a written explanation from a health care provider. 

Please keep in mind: 

  • If you do not meet immunization requirements, you may not be able to engage in the clinical or practicum portions of your program.   
  • Non-medical immunization exemptions are not allowed for most students per Oregon Administrative Rules (OAR).

COVID-19 vaccinations

OHSU strongly recommends that all OHSU members, regardless of their work location or job function, remain “up-to-date” with COVID-19 vaccines, as defined by the CDC and Prevention. Under the revised policy as of June 12, 2023, all students must be “fully vaccinated” or “up-to-date” with COVID-19 vaccinations OR they must submit a declination form via Enterprise Health and adhere to Student Health’s current requirements designed to minimize COVID-19 transmission. New students wishing to submit a declination should do so when they receive their network login credentials (usually about 30 days before the start of your program). 

Confidentiality and reporting of compliance

Student Health and Wellness is charged with overseeing overall campus health. In accordance with federal (FERPA) guidelines and OR SB1558, students submitting the Immune Status Form agree to allow Student Health and Wellness to share compliance-only information with their academic program. Only the designation of compliant or not compliant is shared with your academic program.

If you experience a behavioral health crisis outside of regular hours, call us at 503-494-8665, ext. 5, to talk to a crisis counselor. 

If you need non-emergency medical advice outside of regular hours, call the OHSU hospital at 503-494-8311 and ask for Student Health’s after-hours triage.
